Wars in the 19th Century were accompanied by a very heavy loss of life from infectious diseases. Typhus fever, dysentery, malaria, typhoid fever and yellow fever caused many more deaths than wounds inflicted by enemy actions. During the Peninsular War, for example, for every soldier dying of a wound, four succumbed to disease.

This book examines the development and evolution of surgical practice against this overwhelming risk of death due to disease. It reviews three major conflicts during this time: the Peninsular War, the Crimean War and the Boer War and also considers many minor wars fought by the British Empire in the intervening years, and highlights significant medical and surgical developments during these conflicts.

War surgery in the first part of the 19th Century was brutal and it had to be carried out swiftly. It was performed at speed because there were no anesthetics and the wounded often died during the procedure. Surgeons focused their attention on wounds of the arms and legs, because limbs were both easily accessible to the surgeon (unlike organs inside the abdomen and chest) and lent themselves well to amputation. This was commonly the operation of choice for many war wounds of arms and legs. Some surgeons performed more difficult surgical procedures to try to preserve the limbs and attempted to repair damaged tissues but these operations took longer and caused greater suffering to the patient. Abdominal and chest wounds were not treated since surgeons did not have the means, the ability, or the understanding, to cut into the abdomen and chest to repair the damaged organs successfully.

An important development, which contributed to surgery moving forwards, was the discovery of general anesthesia, which became available in time for the Crimean War. However, whilst it certainly rendered operations pain-free, it was associated with significant numbers of deaths during surgery on wounded soldiers because of the poorly understood effects that anesthetics had, particularly on the heart. As a result, operative surgery did not extend its scope a great deal, and military surgery remained focused on surgery of the limbs. However, fewer amputations were performed during the Boer War at the end of this period.

Britain sent observers to several wars in which it was not involved to learn military lessons and to understand the medical and surgical aspects of war. The American Civil War and the Franco Prussian War were two such conflicts. The Russo Japanese War resulted in a very significant advance in surgery for abdominal wounds, but Western observers either failed to notice or ignored pioneering work performed by a Russian female surgeon called Vera Gedroits. As a result, when the Great War began in 1914, lessons had to be re-learned by British surgeons, and many soldiers who suffered penetrating abdominal wounds lost their lives when they should have survived. Unfortunately, one of the hallmarks of war surgery is that successive generations of surgeons make the same mistakes as their forebears and the same lessons have to be learned time and again.

About the Author
Steve heys was born in Accrington and was educated in England, Australia and Scotland. He graduated in Medicine from the University of Aberdeen in 1981 and undertook his surgical training in the North East of Scotland. He is a Fellow of the Royal College of Surgeons of Edinburgh, Glasgow and England and underwent research training in the Rowett Research Institute in Aberdeen, obtaining a PhD in 1992. He specialised in general and breast cancer surgery for many years before latterly concentrating on breast cancer together with his research interests in the role of nutrition in the causation of cancer. Steve has published more than 200 scientific papers, has written many book chapters on different aspects of surgery and has played many national and international roles in surgery and the provision of surgical services. Steve joins forces with Tom as co-author of this work which explores the development of surgery during the nineteenth and early twentieth century. As Professor of Cancer Surgery at the University of Aberdeen, as well as a former member of the RAMC (V) in the 51st Highland Brigade for 6 years, Steve too is conscious of the major contribution made by Sir James McGrigor both to Army Medical Services and to the University of Aberdeen where McGrigor co-founded the Aberdeen Medical Society and which he supported throughout his long and illustrious career. Tom Scotland - Born in St. Andrews and brought up in the East Neuk of Fife, Tom was educated at Waid Academy in Anstruther. He graduated in Medicine from the University of Edinburgh 1971, becoming a fellow of the Royal College of Surgeons of Edinburgh in 1975. He developed his interest in the Great War whilst a student, when there were still many veterans alive. He trained in orthopaedic surgery in Aberdeen, and after spending a year as a fellow in the University of Toronto, returned to take up the position of Consultant Orthopaedic Surgeon with Grampian Health Board and Honorary Senior Lecturer at the University of Aberdeen. His particular interests were knee surgery, paediatric orthopaedics and tumour surgery, and for three years was lead clinician for the Scottish Sarcoma Managed Clinical network. Over the years he has been a frequent visitor to the Western Front, and has found cycling the best way to visit different places. He has explored many areas of the Western Front with family and friends and since retiring from the National Health Service in 2007 has kept in touch with former colleagues by leading cycling expeditions to the Western Front. He has pursued his interest in the Great War by making a particular study of Aberdeen surgeon, Sir Henry Gray, who played a pivotal role in the development of surgery on the Western Front, and has given various lectures on the development of surgical services during the Great War. In retirement he has completely re-invented himself as a cycling orthopaedic historian.

Drawing on Paul Ricoeur’s theory of metaphor and on Thomas Weiskel’s analysis of the romantic sublime, Grossman illustrates how the novel’s motifs and structures correspond to a closely connected set of ethical, spiritual, political, and aesthetic concerns.

The religious motifs in Les Mis�rables identify the sublime not just with utopian ideals (and the overthrow of Napoleon III’s grotesque Second Empire) but with artistic death and resurrection. Examining the ways the novel is largely concerned with the monstrous "brutalities of progress" called revolutions that must precede the advent of heaven on earth, Grossman traces that link to a mythos of sin and redemption and shows how the moral concerns of the plot also illuminate Hugo’s aesthetics.

Les Mis�rables explores the tensions between heroes and scoundrels, chaos and order, law and lawlessness. Grossman painstakingly follows the novel’s ethical hierarchy from the grotesque (criminality) to the conventional (bourgeois complacency) and the sublime (sainthood), demonstrating how that hierarchy corresponds to two other hierarchies: the literary and the political.

About the Author
NgÅ©gÄ© wa Thiong’o�is an award-winning novelist, playwright, and essayist from Kenya whose novels have been translated into more than thirty languages. He is Distinguished Professor of English at the University of California, Irvine. He lives in Irvine, California.

Namwali Serpell�(introducer) has won the Caine Prize for African Writing and the Rona Jaffe Foundation Writers’ Award. Her debut novel, The Old Drift, is forthcoming from Hogarth. Born in Zambia, Serpell now lives in San Francisco, where she is an associate professor of English at the University of California, Berkeley.

A Whole Mouthful of Torah

Since before the time of Jesus, it has been the practice of observant Jews to read the first five books of the Bible, the Torah, every year. This practice developed into a fifty-two weekly division of the books, each known as a parasha. These readings were later paired with appropriate readings from the prophets. They have served as the basis of synagogue preaching until the present time.

Keren Hannah Pryor’s A Taste of Torah is part of a growing movement that seeks to reconnect Christian faith to its Jewish roots. Pryor writes fifty-two devotions to be paired with the fifty-two weekly Torah readings (as well as the associated prophet readings and a New Testament passage). While Pryor’s reflections only take up a couple pages, the sheer amount of scripture reading will increase the amount of time needed for these devotions.

The scripture reading is both the most fruitful and challenging part of A Taste of Torah. Overall, the devotions themselves were good – particularly for a set of texts that are largely unfamiliar and deemed ‘boring.’ I was also pleased to see occasional Christological connections. However, the best part of the devotion was the practice of reading that much scripture. Often devotions reflect on maybe one or two verses, but Pryor calls for us to engage multiple chapters at a time. This practice itself is worth the price of the book. Yet, it might prove a challenge for some readers. Even though only once a week, A Taste of Torah will require more time and effort than what many are used to from their average Christian devotion.

I highly recommend A Taste of Torah for both personal and family devotions.

Review of 356 Porsche Technical and Restoration Guide, Vol. 2

This book is even better than Vol. 1, published years ago. It is a compendium of articles extracted from the 356 Registry. Typography, layout, and pictures are superior and the articles are invaluable. It is a must for every owner of a 356, bar none. I've got almost every book on the 356 and this is the one that I consult the most. Essential, with absolutely no reservations, and I've been collecting car books for over sixty years.

About the Author
Ralph Hughes, former DW/BI practice manager for a leading global systems integrator, has led numerous BI programs and projects for Fortune 500 companies in aerospace, government, telecom, and pharmaceuticals. A certified Scrum Master and a PMI Project Management Professional, he began developing an agile method for data warehouse 15 years ago, and was the first to publish books on the iterative solutions for business intelligence projects. He is a veteran trainer with the world's leading data warehouse institute and has instructed or coached over 1,000 BI professionals worldwide in the discipline of incremental delivery of large data management systems.

A frequent keynote speaker at business intelligence and data management events, he serves as a judge on emerging technologies award panels and program advisory committees of advanced technology conferences. He holds BA and MA degrees from Stanford University where he studied computer modeling and econometric forecasting. A co-inventor of Zuzena, the automated testing engine for data warehouses, he serves as Chief Systems Architect for Ceregenics and consults on agile projects internationally.

For Leaders: Adapt and Endure

The book is exceptionally well named and, as always from this author, well written. From the always-important perspective of forward-thinking data project leadership, it closes the loop on agile data warehousing at enterprise scale. Books from others have introduced agile dimensional modeling, mapped agile terminology from application development to business intelligence, or defined new, agile enterprise data warehouse architecture in detail. However, this book, which is targeted to project leaders, describes, with sufficient detail for the target reader, how the most challenging, most agile-resistant pieces of data warehousing -- the data structure itself (data model), ETL coding, testing, and change management – are logically broken down into agile-sized, requirements-scoped chunks of business value while completely avoiding data silos, sustaining an enterprise-wide perspective, and as a big bonus, maximizing adaptability to future change and easy enhancement. Friend, you are not dreaming. Following this method, the scope-creep that inevitably followed traditional best practices, from agile requirements to the very non-agile big data-model design up front, becomes a thing of the past.

HYPER NORMAL MODELS: Known to insiders as Data Vault, the methodology is aptly explained at the level of, if you will, the detail oriented project leader or enterprise architect (vs. a data modeler or engineer) seeking a deep immersion for assessment and implementation oversight. Accordingly, the book describes a cross section of architectural alternatives as the method has evolved in Europe, the U.S., and the land down-under. Among these pathways, two important ones that received less coverage than others in this work are Business Vault and Data Vault 2.0 (DV2).

Business Vault, a layer just-downstream containing rules-based transformed data, either as views or tables, has continued to evolve, as it needed to, such that it actually does provide a nicely standardized method for querying the tables once they are loaded into Hyper Normal structure, either for direct querying or in order to subsequently load a data presentation layer. Those who found writing queries directly from a hyper normal model, and especially those getting started as data modelers or ETL engineers, will benefit greatly from learning more about Business Vault patterns.

Data Vault 2.0 is an innovation in Data Vault, intended to move it from primarily a data modeling methodology into a more modern data warehouse logical architecture that supports increased data loading parallelization and a more loosely-coupled integration with Big Data, in which huge data velocity or volume demands an absolute minimum of target table dependencies. The book “Building a Scalable Data Warehouse with Data Vault 2.0”, by Dan Linstedt and Michael Olschimke covers this innovation with the consistency, the detail, and the excellence that it deserves.
However, in my view, it is THIS book by Hughes that provides that all important data project leadership perspective for a data project leadership framework, if you will. Readers of this book who are intrigued with DV2, and who want to succinct, powerful 'how to' on DV2, should look into Kent Graziano’s “Better Data Modeling: An Introduction to Agile Data Engineering Using Data Vault 2.0” (Kindle), which is a near-perfect complement to this one, guiding data modelers and engineers seeking detailed ‘how to’ on DV2 patterns.

Moreover, this book I'm reviewing now has, in my view, the best top-down (project leadership framework) perspective to date on this exciting methodology. Hughes knows how to communicate and make it not just pallatable, but actually compelling.

HYPER GENERAL MODELS: The author also provides a fine introduction to a methodology for transforming source data directly, running it through a commercial transformation engine, then essentially loading it into dimensions and facts -- a proprietary methodology that is described as being available only through a single business intelligence platform vendor. The method, as I read it, does not simply rely, as I had expected it might, on the clever use of well-known table-generalization, aka ‘Universal Data Model’ principles, with their generalized ‘sub-type / super-type’ tables that, as a very high level explanation, reduce the number of tables essentially by storing records of varying concepts in a single table. Notwithstanding the author’s solid coverage of the hyper-general concepts, his indication -- that the commercial platform that actually executes it leaves some of these logical layers of hyper-generalization away from view from designers using the product – leaves me a bit concerned. However, since I have not used the commercial platform, I will not comment on it further, except to say that this author’s explanation of this intriguing platform was excellent.

Practitioners who fail to understand the insights offered in this book – instead sticking with either the old practice of big-enterprise-design-up-front, or the tactical (requirements driven) design, or no design approach of the inexperienced -- will find that their solutions are too slow to adapt to business change in modern times and will soon gather dust. Data warehouse project leaders who read this book and implement its guidance will know how to keep their solutions, careers, and teams dust-free. Recommended reading.

If you want to know about real world Agile Data Warehousing, this is the book to read!

"Agile" is a hot topic these days, but when it comes to data warehousing, being agile is easy to say, but hard to do. Data warehouse projects often get mired down in trying to answer every question that could possibly be asked, rather than answering the questions that are most critical to the business. Ralph makes a great case in this book for a more business value based approach and provides some solid evidence that traditional tool sets have failed to provide the flexibility required. His presentation of the “hyper generalization” approach is very compelling, both in terms of business value and time/cost savings. Nobody knows this area better than Ralph!

I've seen Ralph present this material in person at TDWI, and he's exactly right. 60% to 90% of the labor we currently invest in EDWs today could be eliminated through methods described in this book. I particularly liked this discussion of how alternative data modeling techniques can enable agile delivery methods, and I thought the "hyper generalization" approach he presented had particularly large potential for the data warehouse automation space. Recommend!
